This website collects only the information you choose to type into our contact form: your name, phone number, email address, preferred location, the reason you are contacting us, and your message.

We ask you not to include private medical information, diagnoses, insurance details, Social Security numbers, or other sensitive health information in that form. The form is not a secure channel for medical information.

Information you send is used to respond to your question and, when appropriate, to help our staff arrange an appointment. It is not sold.

This website privacy policy is separate from the practice's Notice of Privacy Practices, which describes how your protected health information is used and disclosed in the course of your care. Ask the front desk for a copy, or call our office.
